In short - friction. Note, that bolt is done up tight. What you may not
have noticed is that the centre of the rubber bush has a steel tube
insert moulded into it. The bolt tightens up on that steel insert, not
on the rubber.

For instance, here is a set of shackle bushes for a Land Rover.

The bush is pressed into the appropriate receptacle in the chassis. Then
the shackle plates and bolts are affixed. The shackle bolt will clamp
the plates hard up against the *inner* steel bush.

A very common arrangement, just need to have the bush in the central
neutral position when tightening. That means you don't tighten the
shackle plate bolts until the vehicle is on its wheels and the
suspension normalised.

Back in the days when I was working on Land Rovers, I had to gas axe
many of those bushes out of the chassis - they rusted in tight as hell.
